Job Description

We are looking for a compassionate and organized individual to provide exceptional care and support to our patients. As a Case Manager, you will play a vital role in coordinating and managing the care of our acute care patients, ensuring they receive the best possible treatment and services.   1. Coordinate and manage the care of acute care patients, ensuring their needs are met and they receive the best possible treatment and services.
  2. Develop and implement individualized care plans for patients, working closely with physicians, nurses, and other healthcare professionals.
  3. Conduct comprehensive assessments of patients' physical, emotional, and social needs.
  4. Collaborate with healthcare team members to identify and address any barriers to patient care.
  5. Monitor patient progress and make adjustments to care plans as needed.
  6. Serve as a patient advocate, ensuring their rights and preferences are respected.
  7. Educate patients and their families about their medical conditions and treatment plans.
  8. Document all patient interactions and interventions accurately and in a timely manner.
  9. Communicate effectively with physicians, nurses, and other healthcare team members to ensure coordinated care.
  10. Stay up-to-date on current healthcare policies, regulations, and standards related to case management.
  11. Maintain patient confidentiality and adhere to all HIPAA regulations.
  12. Participate in quality improvement initiatives and contribute to the development of best practices.
  13. Provide emotional support and counseling to patients and their families.
  14. Collaborate with community resources to ensure seamless transitions of care for patients.
  15. Attend meetings and trainings as required by the healthcare system.

Job Qualifications

  • Bachelor's Degree In Nursing (Bsn) From An Accredited Institution

  • Current And Unrestricted Registered Nurse (Rn) License In The State Of Practice

  • Minimum Of 2-3 Years Of Clinical Experience In An Acute Care Setting

  • Certified Case Manager (Ccm) Credential Preferred

  • Strong Communication, Critical Thinking, And Organizational Skills With The Ability To Work Collaboratively With Interdisciplinary Teams.
