Analyst - Care Planner

We are looking for an experienced Analyst – Care Planner to join CVS Health to assess and analyze patient care plans and health risks. This role requires a proactive and detail-oriented individual to ensure the continuity and effectiveness of care plans. Successful candidates will be able to demonstrate strong problem-solving skills and have a passion for providing excellent patient care.To be successful in this role, you should have a minimum of two years of experience as an Analyst – Care Planner or in a related role. You should also possess excellent communication, organizational, and analytical skills.
