- Serve as the primary point of contact for patients, families, and healthcare professionals to coordinate and manage their care.
- Utilize strong clinical skills and knowledge to assess patient needs, develop care plans, and collaborate with interdisciplinary teams to ensure high-quality care.
- Communicate effectively with patients, families, and healthcare professionals to provide education, support, and guidance throughout the care coordination process.
- Proactively identify and address any barriers to care, such as insurance coverage, transportation, or social determinants of health.
- Utilize electronic health records and other technology to accurately document patient information and track progress towards treatment goals.
- Advocate for patients and their families to ensure their needs are met and their voices are heard in the healthcare system.
- Collaborate with community resources and organizations to provide patients with comprehensive care and support services.
- Monitor and evaluate patient outcomes to identify areas for improvement and make recommendations for best practices.
- Stay up-to-date on healthcare policies, regulations, and advancements to ensure the highest quality of care.

Bachelor's Or Master's Degree In Nursing: A Minimum Of A Bachelor's Degree In Nursing (Bsn) Is Required For A Rn-Care Coordinator Position At Yale New Haven Health System. A Master's Degree In Nursing (Msn) Is Preferred.
Current Rn License In The State Of Connecticut: All Candidates Must Hold A Current And Unrestricted Rn License In The State Of Connecticut To Be Considered For A Rn-Care Coordinator Role.
Minimum Of 3 Years Of Clinical Experience: Candidates Should Have At Least 3 Years Of Clinical Experience As A Registered Nurse, Preferably In A Hospital Or Healthcare Setting. This Experience Should Include Direct Patient Care And Knowledge Of Evidence-Based Practices.
Care Coordination And Case Management Experience: Experience In Care Coordination, Case Management, Or Care Transitions Is Preferred For A Rn-Care Coordinator Role. This Includes Knowledge Of Care Coordination Principles, Discharge Planning, And Utilization Management.
Strong Communication And Organizational Skills: Effective Communication And Organizational Skills Are Essential For A Rn-Care Coordinator At Yale New Haven Health System. Candidates Should Be Able To Communicate Effectively With Patients, Families, And Interdisciplinary Healthcare Teams, As Well As Manage Multiple Tasks And Priorities.
