  1. Provide exceptional and compassionate care to patients facing serious or life-limiting illnesses.
  2. Join the Palliative Care team and work collaboratively with other healthcare professionals to ensure the best possible outcomes for patients.
  3. Conduct thorough patient assessments to develop appropriate treatment plans.
  4. Communicate effectively with patients and their families to discuss treatment options and address any concerns or questions.
  5. Serve as a source of support and comfort for patients and their families during difficult times.
  6. Administer medications and treatments as prescribed by physicians and monitor patient progress.
  7. Maintain accurate and detailed patient records and documentation.
  8. Continuously educate and update oneself on the latest advancements and techniques in palliative care.
  9. Work closely with other departments and healthcare professionals to coordinate care for patients.
  10. Adhere to all hospital policies, procedures, and safety guidelines.
  11. Represent the organization and its mission in a positive and professional manner.
  12. Attend meetings and conferences to stay informed on industry developments and best practices.
  13. Uphold the values of Yale New Haven Health System and promote a culture of inclusivity, respect, and compassion.
  14. Foster a positive and supportive work environment for colleagues and staff.
  15. Maintain confidentiality and adhere to all HIPAA regulations.
  16. Collaborate with other healthcare professionals to develop and implement quality improvement initiatives.
  17. Participate in on-call rotations and respond to emergency situations as needed.
  18. Perform other duties as assigned by the supervisor or department head.

Job Qualifications
  • Medical Degree From An Accredited Institution: A Physician Applying For A Palliative Care Job At Yale New Haven Health System Should Hold A Medical Degree From An Accredited Institution. This Ensures That The Physician Has A Strong Foundation In Medical Knowledge And Skills.

  • Board Certification In Palliative Care: It Is Essential For The Physician To Be Board Certified In Palliative Care To Demonstrate Their Expertise And Proficiency In This Specialized Field. This Certification Is Also Required For Credentialing At Yale New Haven Health System.

  • Experience In Palliative Care: The Ideal Candidate Should Have At Least 3-5 Years Of Experience In Palliative Care. This Experience Should Include Managing Complex Cases, Providing Pain And Symptom Management, And Facilitating Difficult End-Of-Life Conversations.

  • Strong Communication And Interpersonal Skills: As A Physician In Palliative Care, Effective Communication And Interpersonal Skills Are Crucial For Building Relationships With Patients, Their Families, And The Interdisciplinary Care Team. This Also Includes The Ability To Have Difficult Conversations And Provide Emotional Support.

  • Commitment To Patient-Centered Care: Yale New Haven Health System Values Patient-Centered Care, And The Physician Should Demonstrate A Commitment To This Approach. This Includes Understanding And Respecting The Goals And Values Of Patients And Their Families, And Providing Care That Aligns With Their Wishes And Needs.
