1. Provide exceptional patient care by ensuring accurate and timely medication dispensing and counseling.
2. Contribute to the advancement of the field of pharmacy through participation in research projects and staying up-to-date with current developments in the field.
3. Collaborate with experienced pharmacists and physicians to develop and implement patient care plans.
4. Gain hands-on experience in a variety of pharmacy settings, including inpatient, outpatient, and specialty areas.
5. Attend and actively participate in educational and training programs, including lectures, conferences, and workshops.
6. Demonstrate a commitment to continuous learning and professional growth by actively seeking out opportunities to expand knowledge and skills.
7. Communicate effectively with patients, healthcare providers, and other members of the healthcare team.
8. Adhere to all policies and procedures of the Yale New Haven Health System and maintain a high standard of professionalism.
9. Work collaboratively with other healthcare professionals to ensure safe and effective medication use.
10. Contribute to the development and implementation of quality improvement initiatives within the pharmacy department.
11. Act as a role model for pharmacy students and interns by providing guidance and mentorship.
12. Maintain accurate and complete patient records and participate in medication reconciliation processes.
13. Comply with all state and federal laws and regulations related to pharmacy practice.
14. Participate in on-call duties as required.
15. Demonstrate a strong commitment to patient safety and quality care.
16. Continuously evaluate and improve patient care processes.
17. Develop and maintain positive working relationships with colleagues and other healthcare professionals.
18. Maintain confidentiality of patient information and adhere to HIPAA regulations.
19. Complete all required residency program assignments, projects, and assessments.
20. Prepare and present research findings at national conferences and professional meetings.
