Nurse Case Manager

Yale New Haven Health System is seeking an experienced and compassionate Nurse Case Manager to join our team. In this role, you will have the opportunity to make a difference in the lives of our patients. We are looking for an individual who is passionate about healthcare, has excellent organizational skills and is able to provide excellent case management services.The ideal candidate for this position should possess a Bachelor's Degree in Nursing, with current licensure as a Registered Nurse in the state of Connecticut. In addition, at least three years of nursing experience, preferably in a hospital setting, is required. Knowledge of relevant regulations and standards is also necessary.
