  1. Collaborate with healthcare professionals: Work closely with physicians, nurses, and other healthcare professionals to provide comprehensive and coordinated care to patients.
  2. Optimize medication therapy: Utilize your clinical expertise to review medication regimens and make recommendations to improve effectiveness, safety, and cost-effectiveness.
  3. Improve patient outcomes: Use your knowledge and skills to monitor and assess patient responses to medication therapy, identify and resolve drug-related problems, and prevent adverse reactions.
  4. Demonstrate strong clinical background: Utilize your extensive knowledge of pharmacology, drug interactions, and disease states to provide evidence-based and patient-centered care.
  5. Passion for patient care: Show genuine empathy and concern for our patients, and strive to provide the highest quality of care at all times.
  6. Contribute to the healthcare team: Collaborate with other members of the healthcare team to develop and implement medication-related protocols, policies, and procedures.
  7. Adhere to professional standards: Maintain a high level of professionalism and adhere to ethical and legal standards in all aspects of your work.
  8. Stay updated on industry developments: Keep abreast of the latest developments in the pharmaceutical industry, including new drugs and treatments, and apply this knowledge to patient care.
  9. Educate patients and staff: Educate patients and healthcare staff on medication therapy, including proper administration, potential side effects, and drug interactions.
  10. Maintain accurate records: Document all patient interactions, interventions, and assessments accurately and in a timely manner.
  11. Participate in quality improvement initiatives: Contribute to quality improvement efforts by identifying areas for improvement and implementing changes to enhance patient care.
  12. Attend trainings and conferences: Stay updated on the latest developments in clinical pharmacy and attend relevant trainings and conferences to enhance your knowledge and skills.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution.

  • Current And Unrestricted Pharmacist License In The State Of Connecticut.

  • Post-Graduate Residency Training Or Equivalent Experience In A Multi-Specialty Clinical Pharmacy Setting.

  • Board Certification In A Relevant Clinical Specialty (E.g. Ambulatory Care, Critical Care, Oncology, Etc.).

  • Demonstrated Experience And Proficiency In Providing Pharmaceutical Care To Patients With Complex Medical Conditions And Multiple Co-Morbidities.
