We are seeking a compassionate and dedicated Licensed Practical Nurse to join our team at Grimes Center, part of the prestigious Yale New Haven Health System.   1. Provide compassionate and high-quality care to patients and their families.
  2. Administer medications and treatments as prescribed by physicians.
  3. Monitor and document patient's vital signs and medical conditions.
  4. Collaborate with healthcare team members to develop and implement care plans.
  5. Communicate effectively with patients, families, and healthcare providers.
  6. Educate patients and their families about their medical conditions, treatments, and self-care.
  7. Maintain accurate and up-to-date patient records.
  8. Adhere to all safety and infection control protocols.
  9. Assist with admissions, discharges, and transfers of patients.
  10. Respond to medical emergencies and provide appropriate care.
  11. Provide emotional support and comfort to patients and their families.
  12. Participate in performance improvement initiatives and quality assurance activities.
  13. Adhere to all state and federal regulations regarding nursing practice.
  14. Maintain confidentiality of patient information.
  15. Participate in ongoing education and training to maintain current knowledge and skills.
  16. Foster a positive and collaborative work environment.
  17. Serve as a role model for ethical and professional behavior.
  18. Uphold the mission and values of Grimes Center and Yale New Haven Health System.
  19. Continuously strive for excellence in patient care.

Job Qualifications
  • Valid Lpn License: A Qualified Candidate For This Position Must Hold A Valid And Current License As A Licensed Practical Nurse In The State Of Connecticut.

  • Previous Experience: Preferably, Candidates Should Have A Minimum Of 1-2 Years Of Experience As An Lpn In A Healthcare Setting, Particularly In Long-Term Care Or Rehabilitation Facilities.

  • Strong Clinical Skills: The Ideal Candidate Should Possess Strong Clinical Skills, Including But Not Limited To Medication Administration, Wound Care, And Patient Assessment.

  • Excellent Communication Skills: Lpns Are Required To Communicate Effectively With Patients, Families, And Other Healthcare Professionals. Therefore, Candidates Must Possess Exceptional Communication Skills, Both Verbal And Written.

  • Team Player: The Candidate Should Have The Ability To Work Well Within A Team, Collaborating With Other Healthcare Professionals To Provide Comprehensive Patient Care. This Includes Being Able To Take Direction From Supervisors And Work Collaboratively With Other Lpns And Nursing Assistants.
