Job Description

Yale New Haven Health System is seeking an experienced and compassionate Lead Community Health Worker to join our team. In this critical role, you will be responsible for providing health education, health promotion, and community resource navigation to individuals and families in the greater New Haven area. We are looking for someone who is passionate about improving health outcomes and addressing health disparities in our community.This role requires an individual with strong organizational and interpersonal skills, as you will be working directly with both patients and healthcare professionals. Additionally, the Lead Community Health Worker must possess the following qualifications: Bachelor's Degree in Public Health, Health Education, or a related field; at least two years of experience in community health work; and fluency in both English and Spanish.We are looking for someone who is passionate about helping people in our community and eager to join our team of dedicated healthcare professionals.
