
Hospice Registered Nurse- Home Health

We are currently seeking a dedicated and experienced Hospice Registered Nurse to join our Home Health team. - Provide compassionate and comprehensive care to patients and their families facing terminal illnesses.
- Conduct initial assessments and develop individualized care plans for patients in collaboration with the interdisciplinary team.
- Administer medications and treatments as prescribed by the attending physician.
- Monitor and evaluate patient progress and make necessary adjustments to the care plan.
- Educate patients and their families on disease management, symptom control, and end-of-life care options.
- Coordinate with other healthcare professionals, including physicians, social workers, and therapists, to ensure a holistic approach to patient care.
- Provide emotional support and counseling to patients and their families during this difficult time.
- Document and maintain accurate patient records and progress notes.
- Advocate for patient needs and ensure high-quality, personalized care is being provided.
- Participate in interdisciplinary team meetings and contribute to the development of hospice care policies and procedures.
- Continuously update knowledge and skills through participation in educational opportunities and professional development activities.
- Adhere to all legal and ethical standards and maintain patient confidentiality.
- Collaborate with community resources to provide additional support and services to patients and their families.
- Act as a resource and mentor for other team members, such as LPNs and CNAs.
- Adhere to all safety protocols and maintain a safe and clean work environment.

Current Rn Licensure In The State Of Connecticut: The Hospice Rn Must Be A Licensed Registered Nurse In The State Of Connecticut, With An Active And Unrestricted License. This Ensures That The Nurse Is Knowledgeable And Up-To-Date On State-Specific Regulations And Standards Of Care.
Hospice And/Or Home Health Experience: Yale New Haven Health System Requires At Least 1-2 Years Of Experience As An Rn In Hospice And/Or Home Health Settings. This Experience Demonstrates The Nurse's Understanding Of End-Of-Life Care, Symptom Management, And Providing Care In A Patient's Home.
Bls And/Or Acls Certification: Basic Life Support (Bls) And/Or Advanced Cardiac Life Support (Acls) Certifications Are Important Qualifications For A Hospice Rn. These Certifications Ensure The Nurse Is Trained To Respond To Emergency Situations And Provide Life-Saving Interventions.
Strong Communication And Interpersonal Skills: A Hospice Rn Must Possess Excellent Communication And Interpersonal Skills To Effectively Communicate With Patients, Families, And The Interdisciplinary Team. This Includes Empathy, Active Listening, And The Ability To Navigate Difficult Conversations.
Bachelor's Degree In Nursing (Bsn): While Not Always Required, Yale New Haven Health System Prefers Candidates With A Bsn. A Bsn Provides A Strong Foundation In Nursing Theory, Research, And Leadership Skills, Which Are Crucial For Providing Holistic And Evidence-Based Care As A Hospice Rn.
