  1. Provide compassionate and dedicated care to patients and their families in a hospice home setting.
  2. Assist patients with daily living tasks such as bathing, dressing, and feeding.
  3. Monitor and report any changes in patients' condition to the appropriate healthcare professionals.
  4. Administer medication as directed by supervising nurse.
  5. Serve as a source of comfort and emotional support for patients and their families.
  6. Maintain patient confidentiality and adhere to all HIPAA regulations.
  7. Collaborate with other healthcare team members to create and implement individualized care plans.
  8. Maintain accurate and timely documentation of patient care.
  9. Follow all safety and infection control protocols.
  10. Attend ongoing training and education to stay updated on best practices and industry standards.
  11. Uphold and promote the mission and values of Yale New Haven Health System.
  12. Ensure a clean and comfortable environment for patients.
  13. Demonstrate professionalism and empathy in all interactions with patients and their families.
  14. Adhere to all policies and procedures of the hospice home health program.
  15. Contribute to a positive and supportive work environment.
  16. Communicate effectively and efficiently with patients, families, and colleagues.

Job Qualifications
  • Valid Certification As A Certified Nursing Assistant (Cna) Or Home Health Aide (Hha) In The State Of Connecticut.

  • Minimum Of 1 Year Of Experience Working In A Hospice Or Home Health Care Setting.

  • Knowledge Of Hospice Care Principles And Practices, Including Comfort Measures And End-Of-Life Care.

  • Excellent Communication Skills And Ability To Provide Emotional Support To Patients And Their Families.

  • Ability To Work Independently And As Part Of A Multidisciplinary Team, With Strong Time Management And Organizational Skills.
