- Provide compassionate and dedicated care to patients and their families in a hospice home setting.
- Assist patients with daily living tasks such as bathing, dressing, and feeding.
- Monitor and report any changes in patients' condition to the appropriate healthcare professionals.
- Administer medication as directed by supervising nurse.
- Serve as a source of comfort and emotional support for patients and their families.
- Maintain patient confidentiality and adhere to all HIPAA regulations.
- Collaborate with other healthcare team members to create and implement individualized care plans.
- Maintain accurate and timely documentation of patient care.
- Follow all safety and infection control protocols.
- Attend ongoing training and education to stay updated on best practices and industry standards.
- Uphold and promote the mission and values of Yale New Haven Health System.
- Ensure a clean and comfortable environment for patients.
- Demonstrate professionalism and empathy in all interactions with patients and their families.
- Adhere to all policies and procedures of the hospice home health program.
- Contribute to a positive and supportive work environment.
- Communicate effectively and efficiently with patients, families, and colleagues.
Valid Certification As A Certified Nursing Assistant (Cna) Or Home Health Aide (Hha) In The State Of Connecticut.
Minimum Of 1 Year Of Experience Working In A Hospice Or Home Health Care Setting.
Knowledge Of Hospice Care Principles And Practices, Including Comfort Measures And End-Of-Life Care.
Excellent Communication Skills And Ability To Provide Emotional Support To Patients And Their Families.
Ability To Work Independently And As Part Of A Multidisciplinary Team, With Strong Time Management And Organizational Skills.
