Current Rn License In The State Of Connecticut.
Minimum Of 2 Years Of Nursing Experience, Preferably In Home Health Or Community Health.
Strong Clinical Skills And Knowledge Of Disease Management, Wound Care, And Medication Administration.
Excellent Communication And Interpersonal Skills For Effectively Working With Patients, Families, And Interdisciplinary Teams.
Ability To Work Independently And Manage A Caseload, While Also Adhering To Documentation And Reporting Requirements.
