  3. Conduct medication therapy management and provide patient education on the proper use and potential side effects of oncology medications.
  4. Serve as a drug information resource for healthcare providers, patients, and their families.
  5. Participate in multidisciplinary team meetings to discuss patient cases and make recommendations for optimal treatment.
  6. Stay up-to-date on the latest developments and research in oncology, specifically in the area of hematology.
  7. Collaborate with the pharmacy team to ensure proper storage, handling, and preparation of oncology medications.
  8. Document patient care activities and maintain accurate records of medication therapies and interventions.
  9. Adhere to all safety protocols and regulations in handling hazardous and high-risk medications.
  10. Actively participate in quality improvement initiatives to enhance patient outcomes and promote safe and effective medication use.
  11. Provide guidance and mentorship to pharmacy students and residents on rotation in the oncology department.
  12. Contribute to the development and implementation of oncology-related protocols and policies.
  13. Maintain professional competency through continuing education and training opportunities.
  14. Represent the organization in a professional and ethical manner at all times.
  15. Collaborate with insurance companies and third-party payers to obtain coverage for necessary oncology medications for patients.

  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: The First And Most Important Qualification For A Clinical Pharmacist - Oncology (Hematology) Job At Yale New Haven Health System Is A Doctor Of Pharmacy Degree. This Degree Provides The Necessary Knowledge And Training In Pharmacology, Therapeutics, And Patient Care To Effectively Manage Complex Oncology Medications.
