- Provide comprehensive pharmaceutical care to oncology patients, including medication therapy management, drug information, and drug therapy monitoring.
- Collaborate with a multidisciplinary team to develop and implement individualized treatment plans for patients with hematological malignancies.
- Monitor patients for drug interactions, adverse effects, and therapeutic outcomes, making necessary adjustments to medication regimens.
- Serve as a drug information resource for healthcare team members and patients, providing evidence-based recommendations and guidance.
- Stay up-to-date on current developments in hematology and oncology pharmaceuticals, including new drugs, treatment guidelines, and research findings.
- Educate patients and caregivers on proper medication use, potential side effects, and the importance of medication adherence.
- Prepare and dispense medications accurately and efficiently, following all relevant laws, regulations, and safety standards.
- Maintain accurate and complete medication records, including patient profiles, drug information, and adverse reactions.
- Participate in quality improvement initiatives and ensure compliance with all regulatory and accreditation standards.
- Contribute to the development and implementation of policies and procedures related to oncology pharmacy services.
- Train and supervise pharmacy technicians and interns, providing guidance and support as needed.
- Maintain a professional and compassionate demeanor when interacting with patients, families, and healthcare team members.
- Demonstrate a commitment to continuous learning and professional development, attending conferences and continuing education programs as appropriate.
- Adhere to all ethical and legal standards of pharmacy practice.
- Perform other duties as assigned by the Director of Pharmacy or other supervising healthcare professionals.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Program.
Board Certification In Oncology Pharmacy (Bcop) From The Board Of Pharmacy Specialties.
At Least 2 Years Of Experience In A Clinical Oncology Pharmacy Setting.
Strong Knowledge Of Chemotherapy And Other Cancer Treatments.
Proficiency In Using Electronic Health Records And Drug Information Resources.
