  1. Provide comprehensive pharmaceutical care to patients in the oncology and hematology departments.
  2. Work closely with physicians, nurses, and other healthcare professionals to develop and implement treatment plans for patients.
  3. Conduct medication therapy reviews and make recommendations for medication adjustments as needed.
  4. Monitor patients for potential drug interactions, adverse effects, and therapeutic outcomes.
  5. Educate patients on proper medication use and potential side effects.
  6. Collaborate with the healthcare team to develop and implement protocols and guidelines for the safe and effective use of medications in oncology and hematology.
  7. Stay updated on current research and developments in the field of oncology and hematology and apply new knowledge to patient care.
  8. Maintain accurate and complete records of medication orders, dispensing, and interventions.
  9. Ensure compliance with all state and federal regulations related to the practice of pharmacy.
  10. Participate in multidisciplinary rounds and provide input on medication-related issues.
  11. Serve as a resource for other healthcare professionals regarding drug therapy and medication management.
  12. Participate in quality improvement initiatives to enhance patient care and outcomes.
  13. Actively participate in continuing education and professional development opportunities.
  14. Provide guidance and support to pharmacy technicians and other staff members.
  15. Maintain a high level of professionalism and uphold the organization's values and standards.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution

  • Board Certification In Oncology Pharmacy (Bcop) From The Board Of Pharmacy Specialties

  • At Least 2 Years Of Experience In Clinical Pharmacy Practice, Preferably In A Hematology/Oncology Setting

  • Strong Knowledge Of Chemotherapy And Other Oncology Medications, Including Dosing, Side Effects, And Interactions

  • Excellent Communication And Collaboration Skills, With The Ability To Work Effectively Within A Multidisciplinary Team And Provide Patient Education On Cancer Treatments And Supportive Care.
