  1. Collaborate with healthcare professionals to optimize medication therapy and ensure safe and effective medication use for patients.
  2. Utilize clinical expertise and knowledge to assess patient medication needs and make recommendations for appropriate therapy.
  3. Review medication orders for accuracy and appropriateness, and provide drug information to healthcare team as needed.
  4. Monitor patient response to medications and adjust dosages or therapy as necessary.
  5. Identify and resolve medication-related problems and adverse drug reactions.
  6. Educate patients and caregivers on proper medication use, side effects, and potential drug interactions.
  7. Participate in interdisciplinary rounds and provide medication expertise to help improve patient outcomes.
  8. Keep up-to-date with new drugs and therapies, and provide recommendations for formulary additions or changes.
  9. Ensure compliance with all state and federal laws and regulations related to medication use and dispensing.
  10. Maintain accurate and complete medication records and documentation.
  11. Participate in quality improvement initiatives and assist with medication safety initiatives.
  12. Serve as a resource for pharmacy staff and provide guidance and support as needed.
  13. Act as a role model for professional behavior and promote a positive work environment.
  14. Maintain confidentiality of patient information and adhere to all HIPAA regulations.
  15. Continuously strive to improve patient care and actively seek opportunities for professional development and growth.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Or University.

  • Valid License To Practice Pharmacy In The State Of Connecticut.

  • Minimum Of 2 Years Of Clinical Experience In A Hospital Or Healthcare Setting.

  • Board Certification In Pharmacotherapy Or Specialized Area Of Practice (E.g. Oncology, Critical Care).

  • Demonstrated Knowledge Of Current Pharmacy Practices, Regulations, And Patient Safety Measures.
