
Clinical Pharmacist

- Provide exceptional care to patients by utilizing clinical expertise and knowledge of medication therapy.
- Ensure safe and effective medication use by conducting medication reconciliation, drug utilization reviews, and therapeutic drug monitoring.
- Collaborate with healthcare team members to develop and implement individualized treatment plans for patients.
- Educate patients and caregivers on proper medication use, potential side effects, and drug interactions.
- Stay up-to-date with new medication therapies, drug approvals, and evidence-based practice guidelines.
- Maintain accurate and complete patient medication records.
- Participate in multidisciplinary rounds and provide medication-related recommendations.
- Monitor and manage drug inventory to ensure availability and minimize waste.
- Provide guidance and support to pharmacy technicians and other staff members.
- Adhere to all state and federal regulations, as well as hospital policies and procedures.
- Conduct medication therapy management and medication counseling for patients with chronic diseases.
- Collaborate with insurance companies and healthcare providers to ensure appropriate coverage and reimbursement for patients.
- Identify and report medication errors, adverse drug reactions, and other safety concerns.
- Act as a resource for healthcare providers and patients regarding drug information and medication management.
- Participate in quality improvement initiatives and assist with developing and implementing medication-related protocols and guidelines.
- Maintain confidentiality of patient information and adhere to HIPAA regulations.
- Participate in continuing education and professional development activities to enhance clinical knowledge and skills.
- Foster a culture of excellence, compassion, and patient-centered care within the pharmacy department and healthcare system.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program
Active Pharmacist License In The State Of Connecticut
2-3 Years Of Experience In A Clinical Pharmacy Setting
Board Certification In A Relevant Specialty (E.g. Pharmacotherapy, Ambulatory Care, Etc.)
Knowledge And Understanding Of Evidence-Based Practice And Ability To Apply It In Patient Care.
