
Clinical Pharmacist

- Review and evaluate patient medication regimens to ensure safety, effectiveness, and appropriateness.
- Collaborate with healthcare team members to develop and implement medication therapy plans.
- Provide medication education and counseling to patients and their caregivers.
- Monitor and assess patient response to medication therapy and make necessary adjustments.
- Ensure compliance with all relevant laws, regulations, and policies related to pharmaceutical care.
- Participate in medication management and quality improvement initiatives.
- Maintain accurate and up-to-date patient records and documentation.
- Stay informed about new developments and advancements in the field of pharmacy.
- Serve as a resource for healthcare team members and provide guidance on medication-related issues.
- Mentor and supervise pharmacy students and residents.
- Participate in interdisciplinary rounds and meetings to discuss patient care plans.
- Collaborate with other healthcare professionals to develop and implement evidence-based protocols and guidelines.
- Participate in medication reconciliation processes to prevent medication errors.
- Continuously assess and update knowledge and skills through continuing education and professional development opportunities.
- Provide exceptional customer service and promote a positive image of the pharmacy department.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited University.
Active Pharmacist License In The State Of Connecticut.
Minimum Of 2 Years Of Clinical Pharmacy Experience In A Hospital Or Healthcare Setting.
Board Certification In Pharmacotherapy, Ambulatory Care, Or Other Relevant Specialty (E.g. Bcps, Bcacp).
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Healthcare Teams And Educate Patients.
