  1. Conduct comprehensive nutritional assessments for transplant patients, taking into account their medical history, dietary preferences, and current health status.
  2. Develop individualized nutrition plans for patients based on their specific needs and goals, in consultation with the healthcare team.
  3. Collaborate with physicians, nurses, and other healthcare professionals to provide integrated care for transplant patients, ensuring a holistic approach to their nutrition needs.
  4. Monitor and evaluate the effectiveness of nutrition plans and make necessary adjustments as needed to support patients' recovery and overall health.
  5. Educate patients and their families on nutrition and dietary guidelines, including food safety, nutrient requirements, and meal planning.
  6. Stay up-to-date on the latest research and best practices in transplant nutrition, and incorporate new knowledge into daily practice.
  7. Participate in multidisciplinary team meetings and provide input on patient care plans, making recommendations for nutrition interventions as appropriate.
  8. Maintain accurate and timely documentation of patient assessments, nutrition plans, and progress notes in electronic medical records.
  9. Serve as a resource for other healthcare professionals, providing guidance and support on nutrition-related issues for transplant patients.
  10. Adhere to all regulatory and accreditation standards related to nutrition care, including Joint Commission, CMS, and state health department requirements.
  11. Act as an advocate for patients, promoting their nutritional well-being and addressing any barriers to adequate nutrition during the transplant process.
  12. Demonstrate a high level of professionalism and compassion in interactions with patients and their families, maintaining confidentiality and respectful communication at all times.

Job Qualifications
  • Bachelor's Or Master's Degree In Nutrition Or Dietetics From An Accredited Institution.

  • Registered Dietitian (Rd) Or Registered Dietitian Nutritionist (Rdn) Certification From The Commission On Dietetic Registration (Cdr).

  • Minimum Of 2 Years Of Experience In Clinical Nutrition, Preferably In A Transplant Setting.

  • Knowledge Of And Experience With Clinical Nutrition Management For Transplant Patients, Including Nutritional Assessment, Education, And Support.

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate Effectively With Other Healthcare Professionals And Patients.
