  1. Coordinate the care of patients by collaborating with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals.
  2. Develop and implement individualized care plans for patients, based on their specific needs and treatment goals.
  3. Monitor and assess patient progress, identifying any issues or changes in condition and taking appropriate action.
  4. Advocate for patients and their families, ensuring their needs and preferences are considered in all aspects of care.
  5. Provide education and support to patients and their families regarding treatment options, resources, and self-care strategies.
  6. Collaborate with community resources and agencies to ensure patients have access to necessary support services.
  7. Communicate effectively with patients, families, and healthcare team members to facilitate continuity of care.
  8. Utilize evidence-based practices and quality improvement processes to ensure the delivery of high-quality, patient-centered care.
  9. Maintain accurate and timely documentation of patient care, including assessments, interventions, and outcomes.
  10. Stay up-to-date on industry trends, best practices, and regulatory requirements related to patient care management.
  11. Participate in interdisciplinary team meetings and contribute to the development and implementation of quality improvement initiatives.
  12. Serve as a liaison between patients, families, and healthcare providers to facilitate open communication and resolve any conflicts or concerns.
  13. Adhere to all organizational policies, procedures, and standards of care.
  14. Maintain confidentiality of patient information and adhere to HIPAA regulations.
  15. Collaborate with insurance companies and other payers to ensure appropriate authorization and reimbursement for patient care services.

Job Qualifications
