Job Description

Yale New Haven Health System is seeking an experienced Advanced Practice Nurse – Palliative Care to join our team of health care professionals. This is an opportunity to work in a highly collaborative environment where patient-centered care is our top priority. The Advanced Practice Nurse – Palliative Care is an integral part of the health system’s multidisciplinary team, providing direct care to patients and their families, with an emphasis on symptom management, communication, and support.The ideal candidate will have experience in palliative care and a passion for helping patients and their families live out their remaining days with dignity, comfort, and peace. They will be able to effectively communicate with a wide variety of health care professionals, as well as with patients and their families. The Advanced Practice Nurse – Palliative Care will have a strong understanding of the principles of palliative care and must be proficient in the use of evidence-based practice guidelines.Required qualifications include:• Master's degree in Nursing;• Current and valid licensure as a Registered Nurse in the State of Connecticut and in good standing;• National certification as an Advanced Practice Nurse, in a specialty related to palliative care;• At least one year of experience in palliative care.
