1. Provide compassionate and empathetic care to patients and their families through telecare services.
2. Conduct remote assessments and evaluations to determine patient needs and develop individualized care plans.
3. Collaborate with interdisciplinary team members to coordinate and provide comprehensive care to patients.
4. Monitor and manage patients' symptoms and provide appropriate interventions.
5. Educate patients and families about hospice care and assist with end-of-life planning.
6. Maintain accurate and up-to-date medical records and documentation.
7. Communicate effectively with patients, families, and team members to ensure quality care and support.
8. Adhere to all regulatory and compliance standards and follow company policies and procedures.
9. Continuously evaluate and improve telecare services to provide the highest level of care to patients.
