- Lead and manage a team of healthcare professionals to ensure the delivery of high-quality care to patients in their homes.
- Develop and maintain a positive and supportive work environment for team members.
- Oversee and coordinate the daily operations of the home care division, including scheduling, patient assignments, and team meetings.
- Ensure compliance with all state and federal regulations, as well as company policies and procedures.
- Collaborate with other departments and services to provide comprehensive care to patients.
- Conduct performance evaluations and provide ongoing training and development for team members.
- Maintain open communication with patients and their families to address any concerns or issues.
- Monitor team performance and implement corrective action plans when necessary.
- Create and maintain a budget for the home care division.
- Stay up-to-date on industry trends and best practices in home care.
- Participate in strategic planning and goal setting for the home care division.
- Serve as a role model for professionalism, compassion, and dedication to patient care.
- Recruit and onboard new team members as needed.
- Handle any escalated issues or conflicts within the team.
- Foster a culture of continuous improvement and innovation within the team.
Bachelor's Degree In Healthcare Administration, Nursing, Or A Related Field.
Minimum Of 3-5 Years Of Experience In Home Care Management, Preferably In Hospice Or Palliative Care.
Strong Leadership And Communication Skills, With The Ability To Effectively Manage A Team And Collaborate With Other Healthcare Professionals.
Knowledge Of State And Federal Regulations Related To Home Care Services, Including Medicare And Medicaid Guidelines.
Experience In Developing And Implementing Care Plans, Conducting Performance Evaluations, And Managing Budgets.
