1. Provide emotional support and counseling to patients and their families during the end-of-life care process.
2. Conduct assessments to identify social, emotional, and psychological needs of patients and develop individualized care plans.
3. Collaborate with interdisciplinary team to ensure holistic care for patients, addressing their physical, emotional, social, and spiritual needs.
4. Facilitate communication and decision-making between patients, families, and healthcare providers.
5. Connect patients and families with community resources and support services to enhance their quality of life.
6. Advocate for patients and families to ensure their needs are met and their rights are respected.
7. Provide crisis intervention and support during difficult situations.
8. Document all interventions, assessments, and care plans accurately and in a timely manner.
9. Participate in interdisciplinary team meetings, providing input and support for patient care.
10. Continuously educate oneself on current social work practices and end-of-life care strategies.
11. Maintain confidentiality and adhere to ethical standards in all interactions with patients and families.
12. Participate in training and orientation of new team members.
13. Collaborate with community partners and organizations to enhance services and resources available to patients and families.
14. Adhere to all policies, procedures, and regulations of VITAS Healthcare.
15. Foster a supportive and compassionate work environment for patients, families, and team members.
