- Provide medical care and treatment to patients in their homes, ensuring the highest quality of care and comfort.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans for patients.
- Conduct initial and ongoing assessments of patients' medical needs and make appropriate treatment recommendations.
- Monitor and manage patients' symptoms and pain levels, adjusting treatment plans as needed.
- Educate patients and their families on end-of-life care options and support them in making informed decisions.
- Communicate effectively with patients, families, and team members to ensure coordinated and comprehensive care.
- Document all medical care and treatment provided in a timely and accurate manner.
- Stay up-to-date with current medical practices and techniques related to end-of-life care.
- Adhere to all ethical and legal standards in the provision of medical care.
- Demonstrate compassion and empathy towards patients and their families during difficult and emotional times.
- Continuously strive to improve the quality of care provided by participating in quality improvement initiatives.
- Foster a collaborative and supportive work environment by working closely with other team members and promoting a culture of teamwork.
- Maintain confidentiality of patient information and adhere to HIPAA regulations.
- Participate in on-call rotation and provide after-hours care as needed.

Medical Degree: A Physician Home Care Position At Vitas Healthcare Requires A Minimum Of A Medical Degree From An Accredited Institution. This Ensures That The Candidate Has The Necessary Medical Knowledge And Training To Provide Quality Care To Patients.
