1. Provide compassionate and high-quality end-of-life care to patients in accordance with VITAS Healthcare's standards and policies.
2. Support and assist the interdisciplinary team in delivering comprehensive care to patients and their families.
3. Administer medications and treatments as prescribed by physicians and document accurately in patient's medical records.
4. Monitor and report any changes in patients' conditions to the RN Case Manager.
5. Collaborate with the RN Case Manager and other team members to develop and implement individualized care plans for patients.
6. Educate patients and families about their medical conditions, treatments, and self-care techniques.
7. Maintain a safe and clean environment for patients and adhere to infection control protocols.
8. Provide emotional support and comfort to patients and their families during difficult times.
9. Continuously assess and evaluate patients' physical, emotional, and spiritual needs and adjust care accordingly.
10. Participate in ongoing education and training to maintain knowledge and skills required for the role.
11. Communicate effectively with patients, families, and team members to ensure coordinated and efficient care.
12. Adhere to ethical and legal standards and maintain patient confidentiality.
13. Comply with all VITAS Healthcare policies, procedures, and regulations.
14. Act as a positive and professional representative of VITAS Healthcare at all times.
