- Educate patients and families on healthcare processes, procedures, and available resources.
- Advocate for patients and their families to ensure their needs and concerns are addressed by healthcare providers.
- Conduct thorough assessments of patients' healthcare needs and develop individualized care plans.
- Coordinate and schedule appointments, treatments, and other necessary healthcare services for patients.
- Monitor and track patients' progress and provide regular updates to healthcare providers and families.
- Collaborate with interdisciplinary team members to develop and implement strategies for improving patient outcomes.
- Stay up-to-date on healthcare regulations, policies, and procedures to ensure compliance and provide accurate information to patients.
- Maintain detailed and accurate records of patient interactions, care plans, and outcomes.

- Demonstrate compassion, empathy, and professionalism in all interactions with patients, families, and healthcare providers.
Relevant Education And Experience: A Health System Navigator At Vitas Healthcare Should Hold A Bachelor's Or Master's Degree In Healthcare Administration, Nursing, Or A Related Field. They Should Also Have At Least 2-3 Years Of Experience In A Healthcare Setting, Preferably In Care Coordination Or Case Management.
Strong Communication And Interpersonal Skills: As A Health System Navigator, One Must Be Able To Effectively Communicate With Patients, Their Families, And Healthcare Providers. This Includes Active Listening, Empathy, And Clear And Concise Verbal And Written Communication.
Knowledge Of Healthcare Systems And Resources: The Ideal Candidate Should Have A Deep Understanding Of The Healthcare System, Including Insurance Coverage, Medicare And Medicaid, And Community Resources. They Should Also Be Familiar With Electronic Medical Records And Other Relevant Technology.
