1. Conduct initial admissions assessments for hospice patients, including physical, emotional, and psychosocial evaluations.
2. Communicate effectively with patients, families, and healthcare professionals to provide information about hospice services and address any concerns or questions.
3. Collaborate with interdisciplinary team members to develop individualized care plans that meet the unique needs and preferences of each patient.
4. Coordinate and facilitate the admission process, including obtaining necessary medical records and completing required paperwork.
5. Accurately document all patient assessments, interventions, and care plans in electronic medical records.
6. Monitor and assess patient progress, identify any changes in condition, and make appropriate adjustments to care plans.
7. Educate patients and families about the hospice philosophy of care, disease progression, symptom management, and end-of-life planning.
8. Provide emotional support and counseling to patients and families during the admission process and throughout their hospice journey.
9. Collaborate with community resources and healthcare providers to ensure continuity and coordination of care for patients.
10. Adhere to all regulatory and compliance standards related to hospice admissions and documentation.
11. Participate in interdisciplinary team meetings and contribute to the development of quality improvement initiatives.
12. Keep up-to-date on best practices and evidence-based guidelines in hospice care and incorporate them into daily practice.
13. Serve as a patient advocate and promote the VITAS mission and values.
14. Maintain confidentiality of all patient information and adhere to HIPAA regulations.
15. Participate in on-call rotation as needed to provide 24/7 availability for admissions.
