- Provide compassionate and high-quality care to patients and their families during end-of-life journey.
- Administer medications, monitor patient's vital signs, and provide wound care as needed.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Communicate effectively with patients, families, and team members to ensure proper understanding of care plans and patient needs.
- Maintain accurate and up-to-date patient records and documentation.
- Adhere to all policies, procedures, and protocols of the hospice facility.
- Demonstrate empathy and emotional support to patients and families during difficult times.
- Continuously assess patient's physical, emotional, and spiritual needs and provide appropriate care and support.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
- Promote a safe and comfortable environment for patients and their families.
- Maintain confidentiality and respect patient's privacy.
- Advocate for patients' needs and rights.
- Collaborate with community resources and services to ensure continuity of care.
- Provide guidance and support to nursing assistants and other team members.
- Participate in performance improvement initiatives to ensure quality patient care.
- Adhere to all infection control and safety protocols.
- Serve as a resource for patients and families, answering questions and providing information about hospice care.
- Participate in interdisciplinary team meetings and provide input on patient care.
- Maintain a positive and professional attitude towards patients, families, and team members.
- Participate in on-call duties as needed.
Ability To Work Independently And As Part Of A Multidisciplinary Team.
Strong Communication And Interpersonal Skills.
Valid Registered Nurse (Rn) License In The State Of Vermont.
Minimum Of 2 Years Of Nursing Experience, Preferably In Hospice Or Palliative Care.
Bls (Basic Life Support) Certification.
