1. Assess the nutritional needs of patients and develop individualized care plans based on their medical conditions, dietary restrictions, and personal preferences.
2. Collaborate with physicians, nurses, and other members of the healthcare team to provide comprehensive and coordinated nutrition care to patients.
3. Conduct nutrition assessments, including collecting and analyzing patient data, and monitoring progress to evaluate the effectiveness of dietary interventions.
4. Educate patients on proper nutrition and healthy eating habits, including meal planning and portion control, to promote overall wellness and disease prevention.
5. Monitor and document patient progress and make necessary adjustments to dietary plans as needed.
6. Stay up-to-date on current research and best practices in the field of dietetics to provide evidence-based care to patients.
7. Provide guidance and support to patients with chronic conditions, such as diabetes, heart disease, and obesity, to help them manage their conditions through nutrition.
8. Collaborate with food service staff to ensure patients receive appropriate meals that meet their dietary needs and preferences.
9. Maintain accurate and thorough records of patient interactions and treatment plans in accordance with regulatory standards.
10. Participate in multidisciplinary team meetings and provide input on patient care and treatment plans.
11. Maintain professional licensure and certifications, and adhere to ethical and legal standards of practice.
12. Act as a resource and provide guidance to other healthcare professionals on nutrition-related matters.
13. Participate in community outreach programs and educational events to promote healthy eating and nutrition in the community.
14. Continuously evaluate and improve nutrition care processes to ensure the highest quality of care for patients.
