At the UVM Medical Center, we are looking for an enthusiastic Nutrition Care Representative to join our team and provide exceptional care to our patients. As the Nutrition Care Representative, you will be responsible for monitoring patient diets and providing education on healthy eating habits. This is an integral role in ensuring our patients receive the highest quality of care.To be successful in this role, you must be a team player with excellent communication and interpersonal skills. You should have a passion for helping others and a commitment to providing high-quality nutrition care. The ideal candidate will have an educational background in nutrition and/or dietetics, as well as experience in a healthcare setting.We are seeking someone who is organized, detail-oriented, and passionate about helping our patients meet their health and nutrition goals. Responsibilities:

  1. Monitor patient diets and provide education on healthy eating habits.
  2. Work collaboratively with other members of the healthcare team to ensure patients receive the highest quality of care.
  3. Establish and maintain effective relationships with patients and families.
  4. Monitor patient progress and recommend necessary changes in diet plans.
  5. Provide guidance and resources to patients on nutrition and health topics.
  6. Maintain patient charts and records.
  7. Monitor and educate staff on appropriate nutrition protocols and guidelines.
  8. Develop and implement nutrition programs and activities.
  9. Participate in health fairs and other outreach activities.
  10. Stay up-to-date on the latest nutrition research and trends.
