1. Conduct assessments and develop individualized treatment plans for patients with mental health and substance abuse issues, in accordance with hospital policies and procedures.
2. Provide therapeutic interventions, such as individual and group counseling, to patients with mental health and substance abuse disorders.
3. Collaborate with interdisciplinary team members to ensure coordinated and comprehensive care for patients.
4. Maintain accurate and timely documentation of patient progress, treatment plans, and outcomes.
5. Educate patients and their families about mental health and substance abuse disorders, treatment options, and community resources.
6. Advocate for patients and assist with accessing necessary resources and support services.
7. Monitor and evaluate patient progress and adjust treatment plans as needed.
8. Provide crisis intervention and support to patients in acute distress.
9. Facilitate referrals to other healthcare providers and community resources as needed.
10. Participate in multidisciplinary treatment team meetings, case conferences, and other relevant meetings.
11. Adhere to ethical and legal standards in patient care and maintain confidentiality of patient information.
12. Stay updated on current trends, research, and best practices in mental health and substance abuse treatment.
13. Contribute to quality improvement initiatives and participate in program development and evaluation.
14. Collaborate with insurance companies and other third-party payers to ensure appropriate reimbursement for services provided.
15. Maintain professional licensure and participate in ongoing education and training to enhance skills and knowledge.
