- Conduct assessments and develop individualized treatment plans for patients with mental health and substance abuse issues, in accordance with hospital policies and procedures.
- Provide therapeutic interventions, such as individual and group counseling, to patients with mental health and substance abuse disorders.
- Collaborate with interdisciplinary team members to ensure coordinated and comprehensive care for patients.
- Maintain accurate and timely documentation of patient progress, treatment plans, and outcomes.
- Educate patients and their families about mental health and substance abuse disorders, treatment options, and community resources.
- Advocate for patients and assist with accessing necessary resources and support services.
- Monitor and evaluate patient progress and adjust treatment plans as needed.
- Provide crisis intervention and support to patients in acute distress.
- Facilitate referrals to other healthcare providers and community resources as needed.
- Participate in multidisciplinary treatment team meetings, case conferences, and other relevant meetings.
- Adhere to ethical and legal standards in patient care and maintain confidentiality of patient information.
- Stay updated on current trends, research, and best practices in mental health and substance abuse treatment.
- Contribute to quality improvement initiatives and participate in program development and evaluation.
- Collaborate with insurance companies and other third-party payers to ensure appropriate reimbursement for services provided.
- Maintain professional licensure and participate in ongoing education and training to enhance skills and knowledge.
Master's Degree In Social Work, Psychology, Counseling, Or Related Field.
Current Licensure As A Licensed Alcohol And Drug Counselor (Ladc) In The State Of Vermont.
Minimum Of 2 Years Of Experience Providing Mental Health And Substance Abuse Treatment To Individuals And Families.
Knowledge And Understanding Of Evidence-Based Practices For Treating Co-Occurring Mental Health And Substance Use Disorders.
Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team And Effectively Engage Clients In Treatment.
