
Inpatient Pharmacist Clinician
The University of Vermont Medical Center is seeking a dynamic and knowledgeable Inpatient Pharmacist Clinician to join our team. As the Inpatient Pharmacist Clinician, you will be responsible for providing comprehensive and safe drug therapy management to our patients. You will also work collaboratively with other health care providers to ensure the highest quality of care.To excel in this role, you must have exemplary communication, critical thinking, and problem solving skills. You must also possess a doctorate degree in Pharmacy (PharmD) from an accredited college or university, and hold a current license as a pharmacist in the State of Vermont. You must also have 3 to 5 years of experience in a hospital pharmacy setting. Responsibilities:
- Provide comprehensive and safe drug therapy management to patients.
- Collaborate with other health care providers to ensure the highest quality of care.
- Utilize exemplary communication, critical thinking, and problem solving skills.
- Maintain a current license as a pharmacist in the State of Vermont.
- Utilize 3-5 years of experience in a hospital pharmacy setting.
- Maintain knowledge of current trends in clinical pharmacy practices.
- Maintain a positive attitude and demeanour while interacting with patients and colleagues.
- Ensure compliance with all applicable laws, regulations, and standards.
- Monitor and evaluate medication therapies and patient outcomes.
- Document patient encounters, drug interactions, and other pertinent information.
Excellent Communication And Interpersonal Skills
Pharm.d Degree
Current Licensure In The State Of Vermont
Board-Certified Or Board-Eligible In Clinical Pharmacy
Experience In Acute Care Or Ambulatory Care Clinical Pharmacy Services
