
Hematology/Oncology Specialty Pharmacist Clinician
At UVM Medical Center, we are seeking an experienced and motivated Hematology/Oncology Specialty Pharmacist Clinician to join our highly regarded team. This position offers a unique opportunity for an individual to utilize their knowledge and experience in hematology/oncology to provide comprehensive medication management and clinical care to patients. The successful candidate will be well versed in evidence-based practice and will possess excellent communication and collaboration skills.The ideal candidate for this position will possess a Doctor of Pharmacy degree, be licensed as a Pharmacist in the State of Vermont, and have board certification in Oncology Pharmacy (BCOP). Responsibilities:
- Provide comprehensive medication management and clinical care to patients with hematology/oncology needs.
- Utilize knowledge and experience in hematology/oncology to inform care decisions.
- Stay abreast of evidence-based practice and best practices in hematology/oncology specialty pharmacy.
- Maintain licensure and board certification in Oncology Pharmacy (BCOP).
- Provide education to patients and families regarding medications and disease states.
- Source and manage appropriate medications, treatments, and therapies for patients.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure the best possible outcomes for patients.
- Maintain accurate and complete patient records and documentation.
- Assist with the development and implementation of policies and procedures related to hematology/oncology specialty pharmacy.
- Participate in research and development initiatives to improve patient care.
Doctoral Degree In Pharmacy
