At UVM Medical Center, we are seeking an experienced and motivated Hematology/Oncology Specialty Pharmacist Clinician to join our highly regarded team. This position offers a unique opportunity for an individual to utilize their knowledge and experience in hematology/oncology to provide comprehensive medication management and clinical care to patients. The successful candidate will be well versed in evidence-based practice and will possess excellent communication and collaboration skills.The ideal candidate for this position will possess a Doctor of Pharmacy degree, be licensed as a Pharmacist in the State of Vermont, and have board certification in Oncology Pharmacy (BCOP). Responsibilities:

  1. Provide comprehensive medication management and clinical care to patients with hematology/oncology needs.
  2. Utilize knowledge and experience in hematology/oncology to inform care decisions.
  3. Stay abreast of evidence-based practice and best practices in hematology/oncology specialty pharmacy.
  4. Maintain licensure and board certification in Oncology Pharmacy (BCOP).
  5. Provide education to patients and families regarding medications and disease states.
  6. Source and manage appropriate medications, treatments, and therapies for patients.
  7. Collaborate with physicians, nurses, and other healthcare professionals to ensure the best possible outcomes for patients.
  8. Maintain accurate and complete patient records and documentation.
  9. Assist with the development and implementation of policies and procedures related to hematology/oncology specialty pharmacy.
  10. Participate in research and development initiatives to improve patient care.
