- Coordinate and oversee the care of patients with mental health conditions in a multidisciplinary team setting.
- Ensure that patients receive comprehensive and integrated treatment that is in line with their individual needs and goals.
- Collaborate with other healthcare professionals, including psychiatrists, therapists, and primary care providers, to develop and implement treatment plans.
- Serve as the primary point of contact for patients and their families, providing education and support regarding mental health conditions and treatment options.
- Conduct thorough assessments of patients' mental health needs and make appropriate referrals to specialty services as needed.
- Facilitate communication and information sharing between different healthcare providers and agencies involved in a patient's care.
- Monitor and track patients' progress and make necessary adjustments to treatment plans to ensure optimal outcomes.
- Advocate for patients' needs and rights, and assist in navigating the complex healthcare system.
- Maintain accurate and up-to-date documentation of patient care and progress.
- Participate in ongoing education and training to stay current with best practices in mental health care.
- Contribute to quality improvement initiatives and participate in case reviews and team meetings.
- Demonstrate empathy, compassion, and cultural sensitivity in all interactions with patients and their families.
- Adhere to all relevant laws, regulations, and ethical standards in the provision of mental health care.

Master's Degree In Social Work, Counseling, Psychology, Or A Related Field.
Minimum Of 3-5 Years Of Experience In A Mental Health Or Healthcare Setting, With A Focus On Coordinating Care For Individuals With Complex Mental Health Needs.
Knowledge Of Evidence-Based Practices And Treatment Modalities For Various Mental Health Disorders, Including But Not Limited To Depression, Anxiety, And Substance Use Disorders.
Strong Communication And Organizational Skills, With A Demonstrated Ability To Collaborate With Interdisciplinary Teams And Community Partners.
Current Licensure Or Certification In A Relevant Field (E.g. Licensed Clinical Social Worker, Licensed Mental Health Counselor, Etc.) Preferred.
