  1. Coordinate and oversee the care of patients with mental health conditions in a multidisciplinary team setting.
  2. Ensure that patients receive comprehensive and integrated treatment that is in line with their individual needs and goals.
  3. Collaborate with other healthcare professionals, including psychiatrists, therapists, and primary care providers, to develop and implement treatment plans.
  4. Serve as the primary point of contact for patients and their families, providing education and support regarding mental health conditions and treatment options.
  5. Conduct thorough assessments of patients' mental health needs and make appropriate referrals to specialty services as needed.
  6. Facilitate communication and information sharing between different healthcare providers and agencies involved in a patient's care.
  7. Monitor and track patients' progress and make necessary adjustments to treatment plans to ensure optimal outcomes.
  8. Advocate for patients' needs and rights, and assist in navigating the complex healthcare system.
  9. Maintain accurate and up-to-date documentation of patient care and progress.
  10. Participate in ongoing education and training to stay current with best practices in mental health care.
  11. Contribute to quality improvement initiatives and participate in case reviews and team meetings.
  12. Demonstrate empathy, compassion, and cultural sensitivity in all interactions with patients and their families.
  13. Adhere to all relevant laws, regulations, and ethical standards in the provision of mental health care.

Job Qualifications
  • Master's Degree In Social Work, Counseling, Psychology, Or A Related Field.

  • Minimum Of 3-5 Years Of Experience In A Mental Health Or Healthcare Setting, With A Focus On Coordinating Care For Individuals With Complex Mental Health Needs.

  • Knowledge Of Evidence-Based Practices And Treatment Modalities For Various Mental Health Disorders, Including But Not Limited To Depression, Anxiety, And Substance Use Disorders.

  • Strong Communication And Organizational Skills, With A Demonstrated Ability To Collaborate With Interdisciplinary Teams And Community Partners.

  • Current Licensure Or Certification In A Relevant Field (E.g. Licensed Clinical Social Worker, Licensed Mental Health Counselor, Etc.) Preferred.
