
Case Manager - Emergency Department

- Coordinate and manage the care of patients in the Emergency Department to ensure the best possible outcomes.
- Advocate for patients and their families to ensure their needs are met and their voices are heard.
- Collaborate with medical staff, social workers, and other healthcare professionals to develop comprehensive care plans for patients.
- Assess patients' physical, emotional, and social needs and make appropriate referrals for services.
- Communicate effectively and compassionately with patients, families, and healthcare team members.
- Monitor and evaluate the effectiveness of treatment plans and make adjustments as needed.
- Provide education and support to patients and families about their medical conditions, treatment plans, and available resources.
- Document all patient interactions, assessments, and interventions accurately and in a timely manner.
- Stay current on healthcare regulations, policies, and procedures related to case management.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Collaborate with insurance providers and other agencies to ensure patients have access to necessary services and resources.
- Maintain patient confidentiality and adhere to ethical standards.
- Stay up-to-date on advancements and best practices in case management and emergency care.
- Continuously seek opportunities to improve processes and procedures to enhance patient care and satisfaction.
- Foster a positive and collaborative work environment within the Emergency Department team.
Bachelor's Degree In Nursing Or Related Healthcare Field.
Minimum Of 2 Years Experience In Emergency Department Or Critical Care Setting.
Strong Clinical Assessment And Critical Thinking Skills.
Knowledge Of Community Resources And Ability To Collaborate With Interdisciplinary Team.
Certification In Case Management Or Emergency Nursing Preferred.
