- Provide high-quality, patient-centered care to individuals of all ages in the Primary Care department.
- Administer medications and treatments as prescribed by physicians.
- Monitor and record patient vital signs, symptoms, and progress.
- Assist physicians with procedures and examinations.
- Educate patients and their families on health and wellness, disease prevention, and treatment plans.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare team members.
- Maintain accurate and up-to-date patient records.
- Adhere to all hospital policies, procedures, and safety protocols.
- Continuously seek opportunities for professional development and growth.
- Attend and participate in team meetings and educational events.
- Provide support and guidance to other team members, including nursing assistants and medical assistants.
- Adhere to ethical and legal standards of nursing practice.
- Maintain a clean and organized work environment.
- Participate in quality improvement initiatives to ensure the delivery of exceptional care.
- Serve as a patient advocate, ensuring their voice is heard and their needs are met.
- Keep abreast of current trends and advancements in ambulatory care and implement them into practice as appropriate.
- Maintain confidentiality of patient information at all times.
- Provide emotional support and comfort to patients and their families.
- Perform other duties as assigned by the Nurse Manager or other members of the healthcare team.
Ability To Work Independently And As Part Of A Multidisciplinary Team.
Active Lpn License In The State Of Vermont.
Minimum Of 2 Years Of Ambulatory Or Primary Care Nursing Experience.
Knowledge Of Electronic Medical Record Systems, Preferably Epic.
Strong Communication And Patient Education Skills.
