  1. Provide high-quality, patient-centered care to individuals of all ages in the Primary Care department.
  2. Administer medications and treatments as prescribed by physicians.
  3. Monitor and record patient vital signs, symptoms, and progress.
  4. Assist physicians with procedures and examinations.
  5. Educate patients and their families on health and wellness, disease prevention, and treatment plans.
  6. Collaborate with interdisciplinary team members to develop and implement individualized care plans.
  7. Communicate effectively with patients, families, and healthcare team members.
  8. Maintain accurate and up-to-date patient records.
  9. Adhere to all hospital policies, procedures, and safety protocols.
  10. Continuously seek opportunities for professional development and growth.
  11. Attend and participate in team meetings and educational events.
  12. Provide support and guidance to other team members, including nursing assistants and medical assistants.
  13. Adhere to ethical and legal standards of nursing practice.
  14. Maintain a clean and organized work environment.
  15. Participate in quality improvement initiatives to ensure the delivery of exceptional care.
  16. Serve as a patient advocate, ensuring their voice is heard and their needs are met.
  17. Keep abreast of current trends and advancements in ambulatory care and implement them into practice as appropriate.
  18. Maintain confidentiality of patient information at all times.
  19. Provide emotional support and comfort to patients and their families.
  20. Perform other duties as assigned by the Nurse Manager or other members of the healthcare team.

Job Qualifications
  • Ability To Work Independently And As Part Of A Multidisciplinary Team.

  • Active Lpn License In The State Of Vermont.

  • Minimum Of 2 Years Of Ambulatory Or Primary Care Nursing Experience.

  • Knowledge Of Electronic Medical Record Systems, Preferably Epic.

  • Strong Communication And Patient Education Skills.
