  1. Develop and implement population health initiatives to improve the health outcomes of our patient population.
  2. Collaborate with interdisciplinary teams to identify opportunities for improving patient care and reducing healthcare costs.
  3. Provide medication therapy management and education to patients in an ambulatory care setting.
  4. Utilize population health data to identify gaps in care and develop interventions to address them.
  5. Work closely with physicians, nurses, and other healthcare professionals to optimize medication therapy for patients.
  6. Maintain a thorough understanding of current population health trends and best practices.
  7. Serve as a resource to other healthcare providers on issues related to population health.
  8. Participate in quality improvement initiatives to enhance patient outcomes and safety.
  9. Ensure compliance with all relevant laws, regulations, and standards related to pharmacy practice in an ambulatory care setting.
  10. Serve as a leader and mentor to other pharmacists and pharmacy technicians in the ambulatory care population health team.
  11. Continuously assess and improve the effectiveness of population health interventions and programs.
  12. Collaborate with community organizations and partners to promote population health initiatives and engage patients in self-management of their health.
  13. Maintain accurate and complete documentation of patient care activities and interventions.
  14. Stay current on developments in the field of population health and integrate new strategies and technologies into practice.
  15. Comply with all institutional policies, procedures, and protocols relevant to the position.

Job Qualifications
  • Active Pharmacist License In The State Of Vermont

  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Of Pharmacy

  • Completion Of A Post-Graduate Residency Or Fellowship In Ambulatory Care Or Population Health

  • Minimum Of 3 Years Of Experience As A Pharmacist In An Ambulatory Care Or Population Health Setting
