The University of Vermont Medical Center is seeking an ambitious and team-oriented Ambulatory Care Population Health Pharmacist Clinician to join our advanced healthcare team. Responsibilities:

  1. Provide excellent care and services to patients in accordance with the highest standards of clinical population health.
  2. Work collaboratively with other medical professionals to ensure the best quality of care for patients.
  3. Communicate effectively with patients, families, and other healthcare team members.
  4. Maintain current licensure in good standing with the Vermont State Board of Pharmacy.
  5. Remain abreast of new developments in the field of clinical population health and ambulatory care.
  6. Provide superior customer service to patients and their families.
  7. Ensure patient data is accurately entered into medical records.
  8. Adhere to all applicable laws and regulations related to clinical population health.
  9. Participate in quality improvement initiatives to help improve patient outcomes.
  10. Monitor patient outcomes and provide feedback to the healthcare team.
