  4. Conduct regular patient assessments and evaluations to identify and address any potential barriers to care.
  5. Facilitate communication between patients, family members, and healthcare team to ensure all parties are informed and involved in the care plan.
  6. Provide education and counseling to patients and families on treatment plans, medication management, and self-care techniques.
  7. Assist in the coordination of discharge planning and follow-up care.
  8. Maintain accurate and detailed patient records, documenting all case management activities.
  9. Collaborate with insurance companies and other agencies to ensure appropriate coverage and resources for patients.
  10. Stay updated on current healthcare policies, regulations, and standards to ensure compliance in all aspects of care.
  11. Participate in interdisciplinary team meetings to discuss patient cases and provide input on care plans.
  12. Serve as an advocate for patients and their families, addressing any concerns or issues that arise during the course of treatment.
  13. Demonstrate empathy, compassion, and professionalism in all interactions with patients and their families.
  14. Maintain confidentiality and adhere to ethical standards in all aspects of job performance.
  15. Continuously evaluate and improve case management processes and procedures to provide the best possible care for patients.

Job Qualifications
  • Current And Valid Registered Nurse (Rn) License In The State Of California

  • Bachelor's Degree In Nursing From An Accredited Program

  • Minimum Of 3 Years Experience In Case Management, Preferably In A Hospital Or Healthcare Setting

  • Knowledge Of State And Federal Regulations Related To Healthcare And Case Management

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams And Effectively Advocate For Patients.
