Community Health Program Representative

Work at a clinic site identifying and gathering contact information for all patients with high blood pressure within the clinics that meet the inclusion criteria for a research study. Recruit and obtain informed consent from patients eligible to participate in the study. Conduct the collection and data tracking of patient questionnaires and data collection instruments in accordance with study protocol. Assist in the translation of study materials and review for cultural appropriateness. Collect completed data collection instruments, conduct data tracking and maintain current tracking records, and transfer hard data from the UCI clinic site to the office of the Principal Investigator. Assist with facilitation of group discussions with research participants and community stakeholders.
