Social Worker - Outpatient

1. Conduct initial assessments of clients to determine their mental health needs and develop treatment plans accordingly.
2. Provide individual and group therapy sessions to clients with a variety of mental health concerns.
3. Collaborate with other healthcare professionals to ensure comprehensive and coordinated care for clients.
4. Maintain accurate and up-to-date client records, including progress notes and treatment plans.
5. Advocate for clients' needs and assist them in accessing community resources and support services.
6. Provide crisis intervention and support to clients in acute distress.
7. Monitor clients' progress and make adjustments to treatment plans as needed.
8. Educate clients and their families on mental health issues and available resources.
9. Participate in case conferences and team meetings to discuss client progress and treatment plans.
10. Adhere to ethical and legal standards of social work practice.
11. Continuously seek professional development opportunities to enhance skills and knowledge.
12. Collaborate with insurance companies and assist clients with insurance-related issues.
13. Maintain confidentiality and adhere to HIPAA regulations.
14. Work collaboratively with other social workers and mental health professionals to provide a supportive and cohesive team environment.
15. Participate in outreach and community education efforts to raise awareness about mental health services.
16. Follow agency policies and procedures and maintain a safe and professional working environment.
