- Administer medications and treatments as prescribed by the healthcare team.
- Monitor and assess patient's physical and mental health status and report any changes or concerns to the attending physician.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans for patients.
- Provide holistic and compassionate care to patients and their families, ensuring their physical, emotional, and psychosocial needs are met.
- Maintain accurate and timely documentation of patient care, including progress notes, medication administration records, and other required forms.
- Adhere to all facility and state regulations, policies, and procedures related to patient care and safety.
- Communicate effectively with patients, their families, and other healthcare team members to ensure continuity and coordination of care.
- Monitor and maintain medical equipment and supplies, ensuring they are in good working condition and properly utilized.
- Participate in ongoing education and training to enhance clinical skills and knowledge.
- Promote a positive and collaborative work environment by demonstrating professionalism, teamwork, and a commitment to excellence.
- Serve as an advocate for patients, advocating for their rights, needs, and preferences.
- Assist in the orientation and training of new nursing staff, ensuring they are familiar with facility policies, procedures, and standards of care.
- Maintain confidentiality of patient information at all times.
- Adhere to infection control and safety protocols to ensure a safe and healthy environment for patients and staff.
- Demonstrate a commitment to quality improvement by actively participating in performance improvement initiatives and identifying areas for improvement.
Valid Lvn License: The Candidate Must Hold A Valid License As A Licensed Vocational Nurse From The Appropriate State Licensing Board.
Prior Experience In Subacute Care: The Ideal Candidate Should Have Prior Experience Working In A Subacute Care Setting And Be Familiar With The Unique Needs And Challenges Of This Patient Population.
Strong Communication Skills: As An Lvn In A Subacute Care Setting, The Candidate Will Be Responsible For Communicating With Patients, Their Families, And Other Healthcare Professionals. Strong Communication Skills, Both Verbal And Written, Are Essential For This Role.
Attention To Detail: In Subacute Care, Patients Often Have Complex Medical Conditions That Require Close Monitoring And Attention To Detail. The Lvn Must Be Able To Accurately Document Patient Information And Report Any Changes In Condition To The Appropriate Healthcare Team Members.
Compassionate And Patient-Centered Approach: Subacute Care Patients May Require Long-Term Care And Have A Variety Of Physical And Emotional Needs. The Lvn Must Have A Compassionate And Patient-Centered Approach, Providing The Best Possible Care And Support To These Individuals.
