
Intake Specialist, BSW

- Communicate effectively with patients, families, and other healthcare professionals to ensure proper coordination of care.
- Facilitate referrals to appropriate community resources for continued support and treatment.
- Adhere to all policies and procedures related to patient confidentiality and HIPAA regulations.
- Maintain accurate and detailed documentation of patient information and treatment plans.
- Collaborate with interdisciplinary team members to develop and implement individualized treatment plans for patients.
- Educate patients and families about available resources and support services.
- Stay current on industry developments and best practices in the field of social work to continuously improve patient care.
- Coordinate with insurance companies and other payers to verify coverage and obtain necessary authorizations.
- Assist with discharge planning and make appropriate referrals for ongoing care.
- Participate in team meetings and case conferences to discuss patient progress and treatment plans.
- Provide crisis intervention and support to patients and families as needed.
