
Clinical Documentation Specialist

- Review and analyze patient medical records to ensure accuracy and completeness.
- Collaborate with healthcare providers to gather necessary information and clarify documentation.
- Identify and report any discrepancies or missing information in medical records.
- Utilize coding and classification systems to accurately assign diagnoses and procedures.
- Educate and train healthcare providers on proper documentation practices.
- Stay up-to-date on industry regulations and guidelines related to medical documentation.
- Maintain confidentiality and security of patient records.
- Participate in quality assurance and improvement initiatives related to documentation.
- Communicate effectively with interdisciplinary team members to ensure coordination of care.
- Develop and maintain strong working relationships with healthcare providers and other staff.
- Demonstrate a commitment to excellent patient care and outcomes.
- Adhere to all organizational policies and procedures related to documentation and privacy.
- Provide support and guidance to other team members as needed.
- Identify opportunities for process improvement and efficiency.
- Maintain organized and accurate documentation of all tasks and actions taken.
- Attend training and professional development opportunities to continuously enhance skills and knowledge.
- Adhere to strict deadlines and prioritize workload effectively.
- Serve as a resource for colleagues regarding documentation best practices.
- Ensure compliance with all regulatory requirements and standards.
- Perform other duties as assigned by supervisors.
Certified Clinical Documentation Specialist (Ccds) Or Certified Coding Specialist (Ccs) Certification Preferred.
Bachelor's Degree In Healthcare Administration, Nursing, Or A Related Field.
Minimum Of 3 Years Of Experience In Clinical Documentation Improvement Or Medical Coding.
Strong Knowledge Of Icd-10 Coding Guidelines And Documentation Requirements.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate Effectively With Healthcare Providers And Other Team Members.
