  1. Develop and implement individualized care plans for patients based on their needs and goals.
  2. Collaborate with healthcare professionals and other team members to ensure a holistic approach to patient care.
  3. Conduct thorough assessments and evaluations to identify patient needs and potential barriers to care.
  4. Monitor and track patient progress, making necessary adjustments to the care plan as needed.
  5. Facilitate communication between patients, their families, and healthcare providers.
  6. Advocate for patients and ensure that they receive the appropriate services and resources.
  7. Maintain accurate and organized documentation of patient interactions, assessments, and interventions.
  8. Coordinate and schedule appointments, referrals, and follow-up care for patients.
  9. Educate patients and their families about their condition, treatment options, and available resources.
  10. Stay up-to-date on industry standards and best practices in case management.
  11. Assure compliance with all organizational policies and procedures.
  12. Maintain confidentiality and adhere to privacy regulations when handling patient information.
  13. Participate in case conferences and team meetings to discuss patient progress and contribute to treatment planning.
  14. Provide crisis intervention and support for patients and their families in times of need.
  15. Serve as a liaison between patients and external agencies, such as insurance companies and community resources.

Job Qualifications
  • Minimum Of 2 Years Of Experience In Case Management Or Related Field.

  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Ability To Work Independently And As Part Of A Multidisciplinary Team.

  • Strong Knowledge Of Mental Health And Substance Abuse Treatment.

  • Excellent Communication And Interpersonal Skills, With The Ability To Build Rapport With Clients And Stakeholders.
