Job Description

At UNC Health Care, we are seeking a passionate and driven Social Worker to join our team. As a Social Worker, you will be responsible for providing patient-focused care to those in need. You will be an integral part of a team providing services to our diverse patient population, as well as their families, caregivers, and other health care providers.The ideal candidate for this role should have a Master’s degree in Social Work from an accredited school and be currently licensed by the North Carolina Social Work Certification and Licensure Board. You must also have at least one year of post-master's experience in a health care setting. Responsibilities:

  1. Provide patient-focused care to those in need in accordance with UNC Health Care standards
  2. Collaborate with other members of the health care team to ensure positive outcomes for patients
  3. Develop and implement individualized care plans for patients and their families
  4. Monitor and document progress of patients and their families
  5. Provide emotional and social support to patients and their families
  6. Maintain up-to-date knowledge of relevant regulations, social work practices, and resources
  7. Advocate for access to services and resources for patients
  8. Facilitate referrals to appropriate services and resources
  9. Participate in professional development activities as required
  10. Maintain professional licensure and certification
