Job Description

At UNC Health Care, we are looking for an innovative Pharmacy Transition Specialist to join our pharmacy team. The successful candidate will be responsible for providing seamless transitions of care to ensure the highest quality of patient safety.The ideal candidate for this role will have extensive knowledge of the pharmacy profession, exceptional communication skills, and experience with transitions of care. To be successful in this role, you must have:• Bachelor's degree in Pharmacy or a related field• Current North Carolina Pharmacy Technician license• Previous experience as a Pharmacy Technician• Ability to build strong relationships with patients, family members, and healthcare providers• Excellent written and verbal communication skills• Ability to multi-task and work efficiently in a fast-paced environment• Professionalism and commitment to patient safety

Responsibilities:

• Provide seamless transitions of care to ensure the highest quality of patient safety. • Prepare and dispense medications and health care products accurately and efficiently. • Maintain accurate medication records and ensure patient confidentiality. • Demonstrate understanding of the principles of safe medication practice and general pharmacy operations. • Communicate effectively with patients, family members, and healthcare personnel. • Maintain professional pharmacy standards and ensure compliance with laws and regulations. • Monitor and document patient progress, identify problems, and suggest solutions. • Educate patients on proper medication use and drug interactions. • Participate in continuous quality improvement initiatives. • Maintain a safe and clean work environment.
