Welcome to UCLA Health! We are currently seeking a highly motivated and dedicated individual to join our team as an Admin Clinical Care Partner for our Liver Transplant unit on 8 North. In this role, you will play a vital part in providing exceptional patient care and supporting the daily operations of our unit.   1. Greet and welcome patients, families, and visitors to the Liver Transplant unit on 8 North in a friendly and professional manner.
  2. Assist with the daily operations of the unit, including scheduling appointments, coordinating patient admissions and discharges, and managing patient records.
  3. Provide exceptional patient care by responding to patient needs and requests in a timely and compassionate manner.
  4. Collaborate with the healthcare team to ensure the delivery of safe and high-quality care to patients.
  5. Maintain a clean and organized work environment, including stocking and ordering supplies as needed.
  6. Communicate effectively with patients, families, and team members to ensure smooth and efficient functioning of the unit.
  7. Utilize electronic medical records system to input and retrieve patient information accurately and efficiently.
  8. Assist with data entry, data collection, and data analysis as needed for quality improvement initiatives.
  9. Act as a liaison between patients and healthcare providers to ensure effective communication and coordination of care.
  10. Participate in departmental meetings and trainings to stay updated on policies, procedures, and best practices.
  11. Adhere to all UCLA Health policies and procedures, as well as regulatory and compliance standards.
  12. Provide support to other team members as needed to ensure the overall success of the Liver Transplant unit.
  13. Demonstrate a commitment to continuous learning and professional development by attending relevant workshops, conferences, and trainings.
  14. Maintain confidentiality of patient information and adhere to HIPAA guidelines.
  15. Foster a positive and inclusive work environment by promoting teamwork, respect, and diversity.
  16. Represent UCLA Health in a professional and positive manner at all times.
  17. Perform other duties as assigned by the unit manager or supervisor.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field: A Minimum Of A Bachelor's Degree In Nursing Or A Related Field Is Required For This Position. This Will Ensure That The Candidate Has A Strong Foundation In Clinical Care And Will Be Able To Provide Quality Care To Patients.

  • Clinical Experience In Liver Transplant: Prior Experience In A Clinical Setting, Specifically In Liver Transplant, Is Highly Desired For This Role. This Will Demonstrate The Candidate's Understanding Of The Unique Needs And Challenges Of Liver Transplant Patients.

  • Strong Communication And Interpersonal Skills: As An Admin Clinical Care Partner, Effective Communication And Interpersonal Skills Are Crucial For Building Rapport With Patients, Families, And The Healthcare Team. The Ability To Communicate Clearly And Empathetically Is Essential For Providing Quality Care.

  • Knowledge Of Electronic Medical Record Systems: Proficiency In Using Electronic Medical Record Systems Is Essential For Documenting Patient Information Accurately And Efficiently. Experience With Epic, Ucla Health's Electronic Medical Record System, Is Preferred.

  • Ability To Multitask And Work In A Fast-Paced Environment: Working In A Liver Transplant Unit Can Be Fast-Paced And Demanding. The Ideal Candidate Should Be Able To Handle Multiple Tasks Simultaneously And Thrive In A High-Pressure Environment. Time-Management And Organizational Skills Are Key For Success In This Role.
