
Patient Care Assistant - Rehab

- Assist in providing direct patient care under the supervision and direction of a registered nurse or other healthcare professional.
- Monitor and report any changes in patient condition to the nursing team.
- Assist with daily activities such as bathing, dressing, and feeding patients.
- Help patients with mobility and positioning, including transferring and ambulating.
- Take and record vital signs, such as blood pressure, temperature, and pulse.
- Keep patient rooms clean and well-stocked with necessary supplies.
- Collaborate with other healthcare team members to ensure coordinated and efficient patient care.
- Maintain accurate and up-to-date patient records and documentation.
- Provide emotional support and comfort to patients and their families during their stay.
- Adhere to infection control and safety protocols to ensure a safe and healthy environment for patients and staff.
- Participate in team meetings and contribute ideas for improving patient care and department processes.
- Continuously develop and maintain knowledge and skills through ongoing training and education.
- Comply with all hospital policies, procedures, and regulations.
- Demonstrate professionalism and excellent customer service in all interactions with patients, families, and colleagues.
- Assist with other duties as assigned by the nursing team or supervisor.
Certified Nursing Assistant (Cna) License Or Equivalent Certification In Patient Care.
Minimum Of 1 Year Experience As A Patient Care Assistant In A Rehabilitation Setting.
Knowledge Of Basic Medical Terminology And Procedures.
Ability To Effectively Communicate With Patients, Families, And Healthcare Team Members.
Demonstrated Ability To Provide Compassionate And Patient-Centered Care.
